STEP 1
We will need a signed script /referral from your child's primary doctor. The physician can fax the script/referral to us at:

STEP 3
Complete Intake Forms Listed Here or Below (this helps us learn more about your child's needs)
STEP 2
Once the script is obtained by our office, you can call or email us to schedule your child's Evaluation (usually takes 1 hour)
STEP 4
Arrive at least 15-20 mins for appointment.
Don't forget to bring:
insurance card
State ID
FORMS
